Elements and Performance Criteria

1. Elements define the essential outcomes.
2. Plan a community health project
   • Consult key community stakeholders to determine community needs in planning the project
   • Develop project proposals/submissions in response to community consultation
   • Include consultation with key community stakeholders in project planning processes
   • Ensure project plans are in line with community, organisation and legislative requirements
3. Prepare action plan
   • Ensure project plan identifies required actions and resources
   • Document plan and discuss with other workers
   • Identify project risks and assess likelihood and impacts
4. Implement and evaluate project
   • Follow project plan and review in operation to address identified community needs
   • Ensure community is kept informed of progress on the project
   • Discuss project outcomes with supervisors and the community
   • Reflect on project recommendations with the supervisor and community
   • Document project outcomes
   • Evaluate project outcomes and document results

Performance Evidence

The candidate must show evidence of the ability to complete tasks outlined in elements and performance criteria of this unit, manage tasks and manage contingencies in the context of the job role.

There must be evidence that the candidate has:

planned, implemented and evaluated at least one community health project to support the provision of health services for Aboriginal and/or Torres Strait Islander individuals and communities by:

consulting appropriately with community, stakeholders, staff and research team about the purpose, methodology and aims of the research

ensuring confidentiality of community information relevant to the project

performing a project risk assessment

coordinating flow of information with community and research team

facilitating groups for research, discussion and feedback purposes

documenting the progress and outcomes of the project

referring to relevant legislative and organisational requirements as appropriate.

Knowledge Evidence

The candidate must be able to demonstrate essential knowledge required to effectively do the task outlined in elements and performance criteria of this unit, manage the task and manage contingencies in the context of the identified work role.

This includes knowledge of:

relevant Commonwealth, state/territory and local government policies

community/sector networks associated with community health

funding guidelines related to community health projects

local Community Health Plans

organisation policies and guidelines.
