Elements and Performance Criteria

1. Recognise problem
   • Equipment/plant condition is monitored according to enterprise procedures
   • Product is monitored against quality standards and job specifications
   • Conditions/product characteristics indicative of a problem are identified
   • Existing work-based problem and/or practice is recognised
2. Implement quick fix
   • Immediate problem is controlled/contained
   • Quick fix is recommended within scope of competency and authority
3. Determine root cause
   • Range of possible causes are identified
   • Information is gathered to eliminate/confirm causes
   • Assistance is sought as required according to level of responsibility and personal capabilities
   • Root cause is identified based on examination of the above
4. Develop permanent solution
   • Range of methods of eliminating root cause/breaking the cause tree is identified
   • Most appropriate solution is selected based on machinery capabilities, material requirements and job schedules
   • Relevant people are liaised with to confirm decision, if required
   • Solution is recommend/implemented within limits of competency and authority
   • Implementation is monitored and improvements are made as required

Performance Evidence

Evidence of the ability to:

recognise problems and undertake a root cause analysis (RCA), either alone or with assistance, and propose permanent solutions.

Note: If a specific volume or frequency is not stated, then evidence must be provided at least once.

Knowledge Evidence

To complete the unit requirements safely and effectively, the individual must:

explain use of communication, documentation and problem solving techniques

summarise RCA methodology

outline indicators of a problem

identify principles of the process sufficient to undertake a RCA and propose solutions

explain what a 'quick fix' means

outline an analysis in conjunction with relevant analysis tools (cause/effect diagrams, Pareto Charts, 4W).
