Assessment Template

Put assessment names in column headings to track which performance criteria each one covers. Good for ensuring that you've covered every one of the performance criteria with your assessment instrument (all assessement tools together).

2. Select evidence

2.1. Appropriate conforming and/or non-conforming products, materials and/or waste are selected for testing in accordance with enterprise requirements

2.2. Observations of operators and copies of procedures are collected

3. Conduct and interpret tests

3.1. Tests are conducted following workplace procedures and results are documented

3.2. Comparisons of all data collected are made

3.3. Any recommendations are noted and considered for feasibility

4. Report findings

4.1. Findings of tests are documented

4.2. Any potential or existing problems and appropriate recommendations are identified and reported in accordance with enterprise procedures
