Assessment Template
generate a spreadsheet for marking this unit in a classroom environment. Put student names in the top row and check them off as they demonstrate competenece for each of the unit's elements and performance criteria.
Assessment Matrix
a slightly different format than the assessment template. A spreadsheet with unit names, elements and performance criteria in separate columns. Put assessment names in column headings to track which performance criteria each one covers. Good for ensuring that you've covered every one of the performance criteria with your assessment instrument (all assessement tools together).

Elements and Performance Criteria

1. Plan and prepare for fire team operations

1.1 Access, interpret and apply fire team operations documentation and ensure the work activity is compliant

1.2 Obtain, read, interpret, clarify and confirm work requirements

1.3 Identify and address potential risks, hazards and environmental issues and implement control measures

1.4 Select and wear personal protective equipment appropriate for work activities

1.5 Identify and confirm the roles and responsibilities for each team member

1.6 Identify and explain types of fire fighting appliances and confirm their applications

1.7 Identify, establish and maintain communication systems with other personnel during the fighting or containment of fires

1.8 Audit each fire board, depot, sub-station and station, identify, clarify and confirm location and range of appliances

2. Fight or contain fires

2.1 Receive, document, clarify and confirm notification of fire operations from the appropriate authority

2.2 Move or travel to the fire site in accordance with site procedures

2.3 Identify or receive and confirm on the type, nature, source and intensity of the fire and communicate details to appropriate authorities

2.4 Identify, select, wear and apply appliances and equipment appropriate to the fire circumstances

2.5 Continually monitor conditions in the fire area and modify fire fighting techniques/applications to reduce the impact of identified and/or potential hazards

2.6 Avoid unnecessary risks to self and other team members

2.7 Apply isolation and evacuation procedures

3. Finalise the operation

3.1 Avoid fire recurrence by watering, raking down and/or chemical application

3.2 Isolate, barricade, rope-off, secure and monitor fire area

3.3 Clean and inspect appliances and equipment and store in the designated location

3.4 Report, record and process appliances and equipment for maintenance and/or repair

3.5 Participate in debriefs and complete written records/documentation
