Assessment Template
generate a spreadsheet for marking this unit in a classroom environment. Put student names in the top row and check them off as they demonstrate competenece for each of the unit's elements and performance criteria.
Assessment Matrix
a slightly different format than the assessment template. A spreadsheet with unit names, elements and performance criteria in separate columns. Put assessment names in column headings to track which performance criteria each one covers. Good for ensuring that you've covered every one of the performance criteria with your assessment instrument (all assessement tools together).
Wiki Markup
mark up the unit in a wiki markup codes, ready to copy and paste into a wiki page. The output will work in most wikis but is designed to work particularly well as a Wikiversity learning project.

Elements and Performance Criteria

1. Plan and prepare for isolate and access plant

1.1 Access, interpret and apply isolate and access plant documentation and ensure the work activity is compliant

1.2 Identify, record and confirm item of plant, and work to be carried out

1.3 Identify and address potential risks, hazards and environmental issues, and implement control measures

1.4 Select and wear personal protective equipment appropriate for work activities

1.5 Identify and apply written or verbal permit procedure

1.6 Obtain permit clearance before commencing work activity

1.7 Obtain and interpret emergency procedures, and be prepared for fire/accident/emergency

2. Isolate plant

2.1 Resolve coordination requirements with others at the site prior to commencing and during isolation activities

2.2 Establish and maintain a safe working area

2.3 Ensure isolation of all required energy sources and immobilisation of potential energy sources

2.4 Place locks and tags on isolation devices in accordance with the type of permit procedure

2.5 Identify and report breaches in permit procedure safety to authorised personnel

3. Complete permit-to-work form

3.1 Complete statement of isolations

3.2 Complete sign-on and sign-off requirements

3.3 Complete hand-over of plant and equipment

4. Return plant to service

4.1 Resolve coordination requirements with others at the site prior to commencing and during de-isolation activities

4.2 Obtain confirmation that work has been completed and is ready for return to service

4.3 Check designated work to be completed with work/site contract and/or site procedure

4.4 Remove locks and tags from isolation devices

4.5 Restore energy sources

4.6 Identify and correct or report situations, which may endanger individuals or workers

4.7 Confirm all permits are cancelled before plant is brought back into operation
