Undertake basic root cause analysis


Application

This unit describes the skills and knowledge required to contribute to an advanced maintenance strategy.

It applies to individuals who work in an organisation and apply competitive manufacturing strategies. This involves taking responsibility for their work processes, undertaking basic root cause analysis of problems and generally contributing to overall equipment efficiency (OEE).

No licensing, legislative or certification requirements apply to this unit at the time of publication.

Performance criteria describe the performance needed to demonstrate achievement of the element.

1. Recognise problem

1.1 Equipment/plant condition is monitored according to enterprise procedures

1.2 Product is monitored against quality standards and job specifications

1.3 Conditions/product characteristics indicative of a problem are identified

1.4 Existing work-based problem and/or practice is recognised

2. Implement quick fix

2.1 Immediate problem is controlled/contained

2.2 Quick fix is recommended within scope of competency and authority

3. Determine root cause

3.1 Range of possible causes are identified

3.2 Information is gathered to eliminate/confirm causes

3.3 Assistance is sought as required according to level of responsibility and personal capabilities

3.4 Root cause is identified based on examination of the above

4. Develop permanent solution

4.1 Range of methods of eliminating root cause/breaking the cause tree is identified

4.2 Most appropriate solution is selected based on machinery capabilities, material requirements and job schedules

4.3 Relevant people are liaised with to confirm decision, if required

4.4 Solution is recommend/implemented within limits of competency and authority

4.5 Implementation is monitored and improvements are made as required

Evidence of Performance

Evidence of the ability to:

recognise problems and undertake a root cause analysis (RCA), either alone or with assistance, and propose permanent solutions.

Note: If a specific volume or frequency is not stated, then evidence must be provided at least once.


Evidence of Knowledge

To complete the unit requirements safely and effectively, the individual must:

explain use of communication, documentation and problem solving techniques

summarise RCA methodology

outline indicators of a problem

identify principles of the process sufficient to undertake a RCA and propose solutions

explain what a 'quick fix' means

outline an analysis in conjunction with relevant analysis tools (cause/effect diagrams, Pareto Charts, 4W).


Assessment Conditions

Gather evidence to demonstrate consistent performance in conditions that are safe and replicate the workplace. Noise levels, production flow, interruptions and time variances must be typical of those experienced in the Support field of work and include access to:

RCA via simulation or an actual project

an organisation using RCA.

Assessors must satisfy NVR/AQTF assessor requirements.
